6. A statement made under penalty of perjury that the information in your notice is accurate and that you are the copyright owner or authorized to act on behalf of the owner

3. Counter-Notice Procedure

If you believe that content was removed from our site by mistake or misidentification, you may submit a counter-notice containing:

1. Your physical or electronic signature
2. Identification of the removed content and its location before removal
3. A statement under penalty of perjury that you have a good-faith belief the content was removed by mistake or misidentification
4. Your name, address, telephone number, and email address
5. A statement that you consent to the jurisdiction of the Federal District Court in your jurisdiction and will accept service of process

4. Repeat Infringers

In appropriate circumstances, Practivita will terminate the accounts of users who are repeat copyright infringers.

5. Limitation of Liability

Practivita complies with the safe harbor provisions of the DMCA. We are not liable for copyright infringement committed by third parties, provided we respond promptly to valid takedown notices.
